iizuka forms

iizuka is the name of the organisation that hosts the online secure database that we use for recording all the details of our clients, including all meetings / contact with them or on their behalf.  On this page I hope that the videos and information PDFs supplied will help you understand more about the system.

An introduction to iizuka.

Note for Mentors, usually by the time a client is assigned to you the case will have been entered in iizuka with their details and a risk assessment.

The system is quite complex so for ease of use we use five forms:

Pathway Assessment Form.  This is filled in as soon as possible after accepting a client onto the project, it helps to provide a baseline of the client’s needs and position.  Any subsequent changes in any of the fields should be entered but it is not necessary to enter anything in fields that have not changed.

Pathway Assessments Form  It is best to print out this form and complete it with the Client, then enter the details on iizuka later.  Please make sure that you enter any comments, but they do not need to be extensive, note form is acceptable

How to edit a Pathway Assessment  This is a guide on how to edit any saved Pathway Assessment.  It is the same method in principle for editing any other report than the Internal Change Form which unfortunately cannot be ammended once it has been completed.

Internal Change Form.  Again, this is filled in as soon as possible after accepting a client onto the project, it helps to provide a baseline of the clients perception of themselves.  This is a useful tool for identifying areas of need in which a client would benefit from support and encouragement.  There is a visual grid that can be filled in at the same time to help highlight the areas of need.  It is useful to repeat this exercise every 6 to 8 weeks to show any progression as an encouragement for the Client.

Internal Change Form  It is best to print out this form and complete it with the Client, then enter the details on iizuka later.  Please make sure that the Client gives reasons why they feel as they do and enter this in the system.

Internal Change Star blank Print this out and use at the same time as filling out the Internal Change Form, mark the point corresponding to the number value on the Internal Change Form, if you join up the points this gives a spider diagram, the lowest points are areas that need the most support.

Contact Activity Log. Use this for recording all contacts with your client and / or work done on their behalf.

Pathway Action. If you helped them anything that might make a change in any of Pathways topics you should record it as a Pathway Action.

Internal Change Action. If you are working with a client around hope, confidence, making decisions, resilience etc… – complete an Internal Change Action.

Making Changes.

You may at some point need to make changes to some of the Client details or the case details.
